Choosing Dr. Kordestani was the best decision for my augmentation. I had been searching for a plastic surgeon on and off for 3 years. A girl is really feelin herself. Yes it was a bit of an emotional rollercoaster but it's worth it!! They made me aware of all my options, potential risks, and certain activities to avoid before and after my surgery; all important information was given via email as a pdf file.

I would post pictures but I still have about a month left for majority of my inflammation to go down. Pain wasn't so bad but every body varies in pain tolerance so :p

The 6-Step Process of your Surgery


01 Consultation

Complimentary consultation with Dr. Kordestani to discuss YOUR goals, benefits, and risks

02 Measurements

The width of your breasts, also known as the breast footprint, will be measured to help select the right implant. In addition, the shape of your breasts is assessed as well as several other measurements.

03 Surgery date

You select a convenient surgery day. You will go home after surgery, and in most cases your recovery from breast augmentation will take 24 hours.

04 Day of Surgery

You will be seen by myself and your anesthesiologist on the day of surgery. Your breast implant surgery will take approximately 45 minutes.

05 Recovery

You will be in the recovery area for 1 hour before going home. Your recovery at home will be approximately 24 hours. Your chest will feel sore but you will be able to move about. You can resume light exercise in 2 weeks and weightlifting at 6 weeks.

06 Follow-up

You will be seen every 3 months until 1 year postoperatively to ensure you have no complications

Saline

Peace of mind in cases of rupture, which is immediately detectable. The saline fill is not harmful to the body. However, saline feels the least like your own breast tissue and tends not to hold its shape as well as the 2 other types of implants. As a result, it has the most rippling compared to the other implant types, which is most noticeable on the lower-outer portion of the breasts.

Structured saline

Peace of mind in cases of rupture, which is also immediately detectable. Again, saline fill is not harmful to the body. To make saline feel more like breast tissue, the implant has an internal structure so that it feels less like a "water balloon." As a result, it tends to hold its shape well and has less of a tendency to ripple. Overall, structured saline implants feel more natural, but are usually the most expensive implant.

Silicone

Silicone implants are the most popular option. They have a more natural feel than saline and tend to hold their shape better. This results in less rippling. In general, the silicone implants used in cosmetic surgery are more expensive than saline implants but less expensive than structured saline implants. In cases of rupture, the new generation silicone implants are more cohesive, meaning the inner gel still tends to stick to itself. The downside of silicone implants is that it's not as easy to detect rupture as saline implants.

Where is the breast implant placed?

Under breast tissue

Submammary

Also known as the "submammary" plane, this placement technique places the implant under the breast tissue but above the pec muscles. This technique does not provide as much coverage of the implant with your tissue as the other techniques. As a result, the implant edges may be more visible. There's also the possibility that you will see more visible rippling over time. In some studies, it was shown that this technique can result in more cases of abnormal scarring around the implant, also known as "capsular contracture."

Under the muscle

Subpectoral or retropectoral

Also known as the "subpectoral" or "retropectoral" plane, this technique places the implant under the pectoralis muscle. This reduces the rate of "capsular contracture," however, the recovery is more uncomfortable due to the muscle being stretched. The action of the muscle can push the implant downwards and outwards from the breast, which causes the implants to become farther apart over time. Finally, the muscle can prevent the implant from having a well proportioned breast, especially at the lower portion of the breast.

Combination

Dual plane - best of both

The "dual plane" method combines the best of both the submammary and the retropectoral techniques to produce a more natural appearing breast augmentation. The upper portion of the breast is covered by the pectoralis muscle so the implant edge is not visible in clothing. The lower portion has more contact with the tissue above the muscle so that the lower portion of the breast has an appealing shape. The rate of abnormal scarring around the implant is low with this technique and the recovery is much easier than the retropectoral technique.

Smooth shell implants are most commonly used in breast augmentation in the United States

Some surgeons perform breast augmentation with textured shell implants. There are different types of texturing, but in general, they were manufactured with the intent to decrease capsular contracture.

Round


Round implants are the most commonly used in breast augmentation. The "round" refers to the shape when looking at the implant from the front. When looking at it from the side, the back side is flatter than the front.

Shaped


Shaped implants are useful for specific cosmetic cases. They have more "cohesive" or "gummy" silicone that imparts a shape to the implant, with more gel at the lower portion of the implant than at the top. The most commonly used shaped implants are all textured. There is a risk of rotation of the implant which may be significant since it has a distinct shape.
